EveryMind offers case management for homeless adults in Montgomery County including shelter referrals, housing and needs assessments, entitlements and beneficiary referrals, advocacy, education, and resources. Our goal is to connect clients to shelter or permanent housing.Who
Clients must be age 18 and older, without an accompanied minor, actively living on the streets, unsheltered, and not connected to other Homeless Continuum of Care providers.
Referrals
Clients can self-refer, be referred by the community, or by other county providers.

Housing Initiative Program (HIP) Coordination Support Services
Through the Montgomery County Housing Initiative Program (HIP), EveryMind provides service coordination to connect clients to advocacy, education, and resources so they can remain stably housed. Service Coordinators assist with finding housing and support basic needs.Who
Clients must be age 18 and older and formerly homeless and must be referred by the Montgomery County Coordinated Entry System.
Homeless ID & SSI/SSDI Outreach, Access & Recovery (SOAR)
The SOAR program helps clients apply in an expedited process for Supplemental Security Income/Social Security Disability Income (SSI/SSDI). To be eligible, individuals must be 18 or older, be experiencing homelessness or be at risk of homelessness, and have a mental illness, medical impairment, and/or co-occurring substance use disorder.Referrals
